If an original copy is needed, you should request a certified copy from the United States Citizenship and Immigration Services. Access the "Application for Replacement Naturalization/Citizenship Document" at the following link:

Download the N-565 application form. Complete the form as a request for a new certificate of citizenship. You will need to state your reason of request (lost, stolen, or mutilated document; incorrect information; change of name). In the case of damaged or incorrect information, you will need to include the damaged or incorrect copy of the certificate. In the case of lost or stolen forms, you will need to explain the circumstances of the situation.
Include the $345 filing fee and two identical passport-size photographs. The fee must be in the form of check or money order and made payable to either the Department of Homeland Security or to the USCIS.
