How do I now change my name to my married name?

I got married in May 2004 in Knoxville TN. At the time I did not change my name. Now, I live in New York City and would like to change my last name to my husband's. Do I have to use the 'regular' name change kit for New York city, or I can still use the official bride name change kit since the name change is based on nothing else but marriage?

It is recommended to use the New York City forms if you are filing in New York City. A name change for adult package may be used for a new bride. The new bride package contains name change forms for use outside of New York City, and also contains forms such as name change notifications to entities such as employers, banks, etc. However, a notification package may also be puchased separately, as an addition to the name change package, and contains forms and instructions for notifying government agencies and others of a name change.
