What form do I need to assume responsibility for another person's lien?

What form do I need to fill out for this situation. I am in the process of assuming the lien on another individuals commercial truck. I am looking for a form that will clearly state that I will be responsible for the vehicles lien, maintainance, insurance, damage, and all other required permits, licenses, taxes, etc. Is there a document that can be used for this purpose?
