I had my name changed legally in the mid 90's. I lost all my important documents. I now need proof of my name change for a visa related issue. I was born in NYC but had the name changed in New Jersey while I was living there. How do I obtain a copy of my name change?

Answer:
The records of the court order for a name change should be in the filing system at the court where you obtained the name change. I suggest calling the clerk of courts to inquire about obtaining a copy of the order for name change. Often, a copy of a public record may be obtained without the need for an attorney.
